SEABURY VISITING NURSES

Seabury Visiting Nurses (formerly Seabury Home Care) is a not-for-profit, Medicare/Medicaid certified agency that specializes in meeting the home health needs of residents at Seabury and in the surrounding towns. Clients receive care and support in their own home as they continue through the healing process.

 

 

 

 

SERVICES PROVIDED BY SEABURY VISITING NURSES INCLUDE:

 
·         Nursing support and education
·         Case Management of Acute and Chronic Diseases
·         Pain Management
·         Medication Management
·         Wound Care (Wound Vac certified nurses)  
·         PT/INR for anticoagulation therapy              
·         Pre-Discharge Assessment to plan transition to home
·         In-Home Supervision of all personnel
·         Home Safety Evaluations and Fall Prevention
·         Restoring skills following joint replacement                          
·         ADL (activities of daily living) retraining for independence/safety
·         Balance and strength training
·         Adaptive equipment recommendations and training
·         Speech and swallowing evaluations
·         Assistance with available community resources
·         Emergency Response Systems to aid in safety                      
·         Personal Care Assistance
 
 
Criminal background checks and drug screening have been performed on all Seabury employees
 
Medicare, Medicaid, and Private insurances are accepted.
